Your new patient paperwork tells us about the pain and discomfort you are feeling before your initial examination. Please take your time and note even the slightest concern. Auto accident injuries can sometimes worsen over time.
We also ask for some history about your overall health. Some of this paperwork is required and will be shared with your insurance company and/or attorney.
